Fatality inquiry listing for April 2007

Edmonton... Public inquiries held under the Fatality Inquiries Act are limited to establishing the cause, manner, time, place and circumstances of death, as well as the identity of the deceased. The judge may make recommendations on the prevention of similar occurrences but is prohibited, under the act, from making findings of legal responsibility.

 

Calgary

On June 4, 2005, Dale Woodcock was found unresponsive in his cell.

RAFFA, Irene, 80, Calgary

On August 26, 2005, Irene Raffa became short of breath at her nursing home. Emergency Medical Services attended and while she was being transferred to a stretcher she fell and hit her head. She was admitted to hospital and died on August 30, 2005.

On November 16, 2002, Sharla Collier was found in a wooded area a short distance from the group home where she worked.

WARDEN, Jean, 83, Edmonton

On September 11, 2005, Jean Warden was taken to hospital from her
long-term care facility. Her condition deteriorated and she died on
September 23, 2005.

 

*Pre-inquiry conferences are intended to ensure that all parties understand the purposes of the inquiry, what issues will be explored, and what procedures will be followed. No witnesses are called and no evidence is introduced.

 

Edmonton

April 24-27

PERRY, Ronald Ashley, 28, Edmonton
9:30 a.m., Edmonton Provincial Court
Judge F.E. LeReverend

 

 

 

On March 19, 2004, Ronald Perry was arrested for strange and violent behaviour. In an effort to restrain him, he was electronically stunned multiple times by police. After being transported to the hospital he suffered cardiac arrest and died on March 23, 2004.
