Fatality inquiry listing for May 2007

Edmonton... Public inquiries held under the Fatality Inquiries Act are limited to establishing the cause, manner, time, place and circumstances of death, as well as the identity of the deceased. The judge may make recommendations on the prevention of similar occurrences but is prohibited, under the act, from making findings of legal responsibility.

 

Stony Plain

 

May 8-9, 2007

L.S., 17**

On September 28, 2005, L.S. was being transported to his group home when he attempted to jump from the moving vehicle but was prevented. When the vehicle was stopped he ran onto the highway and was hit by a truck.

Lethbridge

On November 16, 2002, Sharla Collier was found in a wooded area a short distance from the group home at which she worked. The cause of death was blunt force injuries to the head. Inquiry May 28-31

Dale Woodcock was found unresponsive in his cell on June 4, 2005. Continuation

On November 28, 2005, Denise Gillies and Alvin Lockhart were found unresponsive after a fire in the Alberta Hotel in Oyen. Continuation

 

Lethbridge

St. Albert

 

May 29, 2007

MOORE, Robert Mervin, 65, St. Albert

Robert Moore fell off a ladder and struck his chest on a cement planter on June 25, 2005. He went to the hospital the next day complaining of chest pain and had a tube inserted, with instructions to return for daily follow-up, which he did. On June 28, 2005 he was brought to the hospital complaining of shortness of breath. He experienced a sudden loss of consciousness, which he quickly regained, and was taken to the Observation Unit where he was found unresponsive later that same day. Continuation

On May 8, 2006, Eva Farnel was found unresponsive hanging in her cell. She died in hospital on May 12, 2006. Inquiry October 29-30

 

Siksika

On January 2, 2006, D. B. was found suspended by her neck in the basement of her home.
